ECFMG Certification & the Pathways
Certification is the gate every international medical graduate passes through before U.S. residency — and it is a process with hard deadlines, not a single exam. This is the map: the four requirements, the six Pathways, the English test, and the clocks you cannot let lapse.
What it is, and why you need it
ECFMG Certification is the standard for evaluating whether an international medical graduate is ready to enter U.S. graduate medical education; ECFMG — a division of Intealth — evaluates your qualifications and issues the certificate. (Since January 2026, USMLE registration and score services for international students and graduates are handled by the FSMB, not ECFMG; as you register for each Step, FSMB confirms your ECFMG-certification eligibility.) You need the certificate for four things:
- Residency. The ACGME requires IMGs entering accredited residency or fellowship programs to be ECFMG-certified before they start.
- USMLE Step 3. Certification is one of the eligibility requirements for IMGs to sit Step 3.
- Licensure. Most state boards require it (among other things) for an unrestricted license, though the exact rules vary by state.
- A J-1 visa. ECFMG (Intealth) is the U.S. Department of State–designated sponsor of J-1 exchange-visitor physicians; a valid certificate is a prerequisite for sponsorship. (J-1 physicians carry the two-year home-residency requirement — see J-1 waiver paths.)
The four requirements
Certification has four parts. You can work on them in parallel, but all four must be complete — and two of them run on clocks.
1 · Application for ECFMG Certification
You submit an Application for ECFMG Certification (it confirms your intent and collects your medical-education details) before ECFMG will verify credentials or process a Pathway. It must be in accepted or pending status for you to apply to a Pathway.
2 · The examinations
Pass USMLE Step 1 and Step 2 CK. Step 1 has been reported pass/fail only since January 26, 2022; Step 2 CK still returns a three-digit score. Watch the clock:
3 · Medical education credentials
Two things must be true. First, your medical school must be listed in the World Directory of Medical Schools with an ECFMG Sponsor Note stating it meets ECFMG's eligibility requirements. If there is no Sponsor Note on your school's listing, you are not eligible for certification at all — check this first. Second, you must have been awarded credit for at least four credit years, and ECFMG must verify your final diploma directly with the school (see Credentials & verification).
4 · Clinical and communication skills
You meet this either by completing an ECFMG Pathway — which bundles a satisfactory OET Medicine score — or by holding a passing performance on the former USMLE Step 2 CS that is still valid for certification. Step 2 CS has been discontinued, so for almost everyone applying today, the route is a Pathway.
The six Pathways
The Pathways are how you satisfy the clinical-skills requirement without Step 2 CS. There are six for the 2027 cycle. You don't freely pick a Pathway. If you meet Pathway 1's licensure criteria you must use it — unless you have ever failed Step 2 CS, which routes you to Pathway 6 regardless of anything else. If you're not Pathway 1–eligible, your MyIntealth application automatically determines your eligibility for Pathways 2–5 from your record; if none applies, you use Pathway 6. Every Pathway also requires a satisfactory OET Medicine score — no exceptions.
| Pathway | Who it's for | Key cutoff | What proves it |
|---|---|---|---|
| 1 | Already licensed to practice medicine without supervision in any country | License valid on/after January 1, 2022 | Certificate/Letter of Good Standing, issued within 90 days, sent directly by the authority |
| 2 | Passed a licensure OSCE required by your country's medical regulator for all students | Graduated on/after January 1, 2024 | School's OSCE Attestation (students / no diploma yet) or diploma verification (graduates); the regulator's OSCE requirement predates 2020 |
| 3 | School accredited by a WFME-recognized agency | Graduated on/after January 1, 2024 | Clinical Skills Attestation from an authorized school official |
| 4 | School accredited by an agency with NCFMEA comparability | Graduated on/after January 1, 2024 | Clinical Skills Attestation from an authorized school official |
| 5 | MD granted jointly with a U.S. LCME-accredited school | Graduated on/after January 1, 2024 | Clinical Skills Attestation from an authorized school official |
| 6 | Everyone who doesn't meet 1–5 (and Step 2 CS failers) | — | Six real, in-person clinical encounters scored on the ECFMG Mini-CEX |
Pathways 3–5 all rest on the same act — an authorized official of your medical school attests to your clinical skills — and differ only in how your school is accredited. They are open to students and recent graduates alike.
The English requirement — OET Medicine
Every Pathways applicant takes the Occupational English Test (OET) — Medicine, regardless of native language, language of medical instruction, or citizenship. There are no exceptions and no waivers. You must hit every minimum in a single sitting:
| Sub-test | Minimum score |
|---|---|
| Listening | 350 |
| Reading | 350 |
| Speaking | 350 |
| Writing | 300 |
Miss even one sub-test and you retake all four — there is no banking of individual sub-scores. For the 2027 cycle the score must be attained on or after January 1, 2025, and ECFMG recommends testing by the last scheduled December 2026 date to have results in time for the Match. Budget separately for it: OET is not an ECFMG fee, and OET lists a price of roughly US$455 (set by OET, subject to change — confirm when you book). (ecfmg.org)
Credentials & verification
ECFMG verifies your medical credentials by primary-source verification — confirming your final diploma and transcript directly with your medical school, not from the copies you provide. You submit them through the MyIntealth platform as part of your Application for ECFMG Certification.
Deadlines & fees (2027 cycle)
- Application & detailed info available: since May 15, 2026.
- Pathways application deadline: January 31, 2027 (Eastern Time) — and required documentation / scores must be received by then.
- Pathway 1 Good-Standing letter: received by January 31, 2027.
- Pathways 3–5 electronic Clinical Skills Attestations and Pathway 6 Mini-CEX evaluations: by February 15, 2027 (paper attestation forms by January 31).
- OET Medicine: test by the last December 2026 date; score valid if attained on/after January 1, 2025.
Fees (ECFMG's mid-2026 figures — always confirm current on ecfmg.org/fees): Application for ECFMG Certification $580; credential verification $220 ($110 diploma + $110 transcript); Pathway application $945; ERAS token $185. OET is separate (~US$455, paid to OET, not ECFMG). Not everyone pays every item at once, and USMLE exam fees are billed separately by the FSMB.
Validity, revalidation & the Match
Not to be confused with ABIM’s Pilot Pathway E — a board-certification route for physicians who completed a U.S. fellowship without a U.S. residency. Different body, different stage, unrelated to these Pathways. See Fellowship Without a U.S. Residency.
A certificate earned through a Pathway expires when that Pathway expires: the 2027 Pathways certificate is valid through December 31, 2029 (each cycle is a two-year-plus window). A certificate based on a still-valid Step 2 CS pass does not expire.
You make certification permanent — no longer subject to expiration — by any one of: completing at least 12 months of ACGME-accredited U.S. GME; completing 12 months in an approved non-standard training program tied to an ACGME program; or holding an unrestricted U.S. medical license. An expiring Pathway can also be revalidated.
Sequencing & common mistakes
- Check the World Directory Sponsor Note first. No Sponsor Note means you are ineligible — find that out before spending a dollar or a month.
- Start your credential verification early. It runs through MyIntealth as part of your Application for ECFMG Certification and depends on how fast your school responds — which you don't control. (EPIC is a separate service and is not required for certification.)
- Take OET before the window closes, and know that one weak sub-test means retaking all four.
- Pick the right Pathway. Mind the cutoffs — licensure valid on/after January 1, 2022 for Pathway 1; graduation on/after January 1, 2024 for Pathways 2–5. Step 2 CS failers use Pathway 6.
- Respect the seven-year exam clock — no reminders are sent.
- Certification must be complete before residency begins, and your exam, clinical-skills, and communication-skills requirements met and verified by the NRMP ROL deadline to rank.
